Whether you're a returning family or new family joining our team, the registration process is simple and takes only a few minutes. You will be requested to pay during the last step of the registration process. At the conclusion of the registration process you will be granted access to your own private account for the team.

You private account is your one-stop-shop for maintaining your contact information, declaring for swim meets, signing up for jobs, interactively tracking your kids times, and much more. We look forward to having you be part of our incredible swim team.

Liability Waiver

By registering my child(ren) with the Sauk Prairie Pool Sharks, I agree to participate (or allow my child(ren) and family members to participate) in the Sauk Prairie Pool Sharks, and hereby release Sauk Prairie Pool Sharks, its directors, officers, agents, coaches, and employees from liability for any injury that might occur to myself (or to my child(ren) and family members) while participating in the Sauk Prairie Pool Sharks program, including travel to and from training sessions, swim meets or other scheduled team activities.


I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members) participating in the Sauk Prairie Pool Sharks program.

PARENT & ATHLETE AGREEMENT

As a Parent and as an Athlete it is important to recognize the signs, symptoms, and behaviors of concussions. By signing this form you are stating that you understand the importance of recognizing and responding to the signs, symptoms, and behaviors of a concussion or head injury and certify that you have read, understand, and agree to abide by all of the information contained in this sheet. You further certify that if you have not understood any information contained in this document, you have sought and received an explanation of the information prior to signing this statement.

Parent Agreement:

Code of Conduct Form

SAUK PRAIRIE POOL SHARKS SWIMMER CODE OF CONDUCT

The swimmers themselves are ultimately responsible for their own behavior. Sauk Prairie Pool Sharks (SPPS) is a swim team, and all swimmers must respect themselves, their team, and their coaches. The Head Coach is ultimately responsible for helping reinforce the code of conduct; the swimmers themselves are responsible for following it.

All SPPS swimmers are expected to be polite, cooperate with and respect others and all property. They should demonstrate good behavior when attending practices and meets ~~ in and out of the pool~~ as well as at all SPPS activities. Swimmers will not embarrass, ridicule or demean others under any Circumstances. Any swimmer that endangers another swimmer, disrupts practices or meets, or makes insulting or derogatory remarks, gestures or acts (including taunting directed at coaches or other swimmers) will be deemed to be not following this code of conduct.

All pool safety rules are clearly posted in the pool area and must be followed. Always put safety first, last and always. No one is allowed in any part of the pool until permission comes from a coach on deck. No diving or jumping into the pool unless under direct supervision of a coach. No food, gum or beverages are allowed in the pool area. Spitting or sharing of any body fluids is not tolerated in the pool, pool area or in the locker rooms.

Any swimmer found or suspected of any activity listed above will be dealt with by the Head Coach as described:

1. The coach will give the swimmer a verbal warning.

2. If there is a second occurrence, the coach will contact the parent to seek their assistance in dealing with the behavior.

3. If there is a third occurrence, the coach will confer with the Board of Directors which may result in the swimmer being dismissed from the team at their discretion. No fees will be refunded to dismissed members.

SPPS has a zero tolerance policy towards drugs. Swimmers are not to use or possess alcohol, tobacco, illegal drugs, or other banned substances. This includes the use of performance enhancing substances such as caffeine pills, creatine, steroids, etc.

Volunteer Fee

Every summer, Pool Sharks hosts 3 or 4 home meets and we rely on parents to volunteer at each one. At least one family member must volunteer for a job at each home meet their child swims. This is a $100 conditional fee per family, to be applied during registration but not charged during checkout.  At the end of the season, if volunteer requirements are fulfilled, this conditional fee will not be charged to your credit card. If they are not fulfilled, the conditional fee will be charged and deposited into the Pool Sharks General Fund for purchase of new equipment for the Pool Sharks program.

There's a $20 concession fee per family to purchase supplies for home meets.

Sauk Prairie Pool Sharks has my permission to use my or my child’s photograph or other media images such as videos to promote the the teams interests. I understand that the images and videos may be used in print publications, online publications, presentations, websites, and for social media purposes. I also understand that no compensation shall become payable to me for such use. Please only check below if you do not give permission.
